Job Description

The Eligibility & Enrollment Specialist (EES) is responsible for assisting patients and potential patients with applications for benefits including YNHH Free Care, Access Health CT, Medicaid, the CS-HHC Sliding Fee Discount, and other available programs. The EES help guide applicants through the process, clarifies terms, addresses literacy barriers, and identifies required documentation. The EES submits applications, tracks progress to final determination and maintains accurate records. On occasion, the EES may also assist with patient complaint resolution under applicable CS-HHC policies and procedures.

Responsibilities will include but are not limited to:

  • Attain expertise and maintain knowledge of available benefit programs, including Access Health CT, Medicaid, other State assistance programs, CS-HHC Sliding Fee Discount, and YNHH Free Care Programs.
  • Responsible for screening potential applicants for benefit eligibility and making referrals for additional services as appropriate.
  • Provide excellent customer service by guiding applicants through the application process from start to finish.
  • Obtain and maintain Access Health CT Certification as a Certified Application Counselor.
  • Submit completed applications, advocate with DSS when necessary, and track applications to final determination.
  • Maintain meticulous and confidential records of all applications and outcomes.
  • Input data and generate reports as required.
  • Must be flexible on evenings and weekends to attend community outreach, including schools, health fairs, and other venues.
  • When needed, assist with patient complaint resolution under CS-HHC policies and procedures.
  • Perform other related duties as assigned.

Requirements:

  • High School Diploma/GED required.
  • Familiarity with Medicaid, Access Health CT, and Sliding Fee Discount programs required.
  • Experience with Microsoft Office is strongly preferred.
  • Experience with Epic preferred.
  • Ability to communicate coverage policies to patients. Strong advocacy, organizational, data entry and computer skills required.
  • Ability to relate to diverse populations and manage conflict diplomatically.
  • Excellent attention to detail and record keeping
  • Bi-lingual (English/Spanish) speaking highly desirable
