Community Bridges, founded in 1982, is a non-profit agency with a mission to advance the integration, growth and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access and find creative solutions to develop meaningful community engagement.
We are currently looking for 1 Skills Trainer for the Community Support Services Department in Concord, NH!
The Project Search Skills Trainer's responsibility is to work in collaboration with the Project Search Teacher and provide assistance to interns participating in the program whom may have barriers to employment. This program takes place in the hospital setting and follows the Concord school districts school year. The position includes working with the Project Search interns in both a classroom and vocational setting. The skills trainers will be responsible to work alongside the Teacher to network within the hospital to identify new internships, aid in break down and simplifying of job tasks to better assist interns in learning these roles, assessing vocational abilities and safety skills as well building upon employability skills. The position will also entail coordination of career activities including; interviewing, filling out job applications, resume writing, and assistance with job placement and on the job supports. On the job supports include role modeling, problem solving, interpersonal skills, and mentoring the individual to create success career paths. Skills Trainers are also expected to work collaboratively with Community Choices SEP Manager and Lead Job Developer to secure employment for interns participating in the Search program.
Qualifications:
Education: High School Diploma or GED
Experience: Experience in the human service or related field with persons with disabilities desirable skills in coaching, mentoring, or vocational also desirable.
Compensation:
There is a $1000 sign on bonus for this position - $500 will be paid at time of hire and $500 will be paid after 6 months in the role.
Paid Time Off: For a full-time employee working 40 hours a week, 22 days of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!
Mileage Reimbursement: Employees are reimbursed for approved mileage driven on the clock.
Paid Training: As an employee, you are paid your usual rate of pay while attending trainings!

Current Open Positions Available:
Part-Time: Monday through Friday (7am-3pm, during the school year only)
Specific Duties and Responsibilities:
- Participates in basic Job Coach training and continual staff development.
- Performs job/task analysis of the worksites and competitive jobs.
- Teaches the essential tasks of the job and reinforces employability skills.
- Assists in training students in interview process (unique to each host business) in order for students to gain both work sites and competitive positions and provides support to interns to understand the job and duties required.
- Learns the job and makes any modifications necessary to the successful completion of the job. Each Skills Trainer is assigned interns and will spend time with individual interns wile on the work site. Due to the goal of independence, Skills Trainers will reduce the amount of time spent with each intern as skills are developed.
- Attends job orientation with the intern and clarifies information with the intern as necessary.
- Maintains a positive and communicative business relationship with Project Search Teacher and other members of the Community Choices team.
- Works with instructor to assess student skills and gather baseline data.
- Works with business liaison, department managers, teacher and fellow job coaches to develop internship sites, write job descriptions, task lists, job analysis and plan for necessary modifications for the internships and competitive jobs.
- Works with teacher, managers, co-workers and community to educate on disability awareness and eliminate preconceived ideas and stereotypes about persons with disabilities.
- Reinforces employable skills and assists intern to understand and interpret work culture
- Teaches safe practices according to the work environment, models professional workplace behavior and support individuals to learn the skills.
- Works with Teacher to determine new skills as intern gains competencies and confidence.
- Meets with the Teacher and SEP Manager regularly to discuss issues and solve problems.
- Assesses student progress and give feedback to intern and team.
- Completes necessary evaluations, reports and other documentation.
- Assists Teacher in networking within the hospital as needed in attempt to increase departmental internships within the hospital.
- Function under the understanding that Project Search's goal is to have 100% employment rate each year. Having open dialogues with team on improvement/changes that are necessary to help us get to 100% employment each year.
- Attend monthly SEP team meetings.
- Assist program manager and Lead Job Developer as needed during the transition to Community Choices Job Coaches once interns have graduated.
- Support individuals on the job site ensuring proper mentoring and success in the role based on the job role the individual will be performing.
- Promote independence with job functions and provide skills training for how employees perform successfully on the job
- Help design plans and work with employee's onsite to create a successful fade program and be a resource to the job site for ongoing support.
- Provide individualized supports to assist individuals in establishing, maintaining, and presenting a positive image. to include, but not be limited to, direct support, personal care and/or skill acquisition etc.
- Provides supports while considering the preferences of individuals. Actively encourage choice making.
- Assist individuals in establishing, maintaining, and presenting positive images. In so doing, assist with toileting, bathing, showering, personal and dental hygiene, etc.
- Assist in developing the Individual Service Plan; provide information for assessments; develop implementation strategies for goals/ objectives; monitor the delivery and quality of supports.
- Complete incident reports, data collection, probes, medication logs, and individual data accurately and in a timely manner.
- Demonstrate an awareness of and respect for each person's legal, civil, and human rights-including maintaining confidentiality of information regarding the individual. Ensure the respect and dignity of persons with developmental disabilities.
- Attend team meetings and agency meetings as assigned.
- Attend trainings and seminars as required.
- Report any safety, health or human rights concern immediately to the Supervisor.
- Perform all delegated responsibilities, which may include filing, updating data collection tasks, and other activities related to the provision of supports and services.
- Perform Administrative Assistant / Program Coordinator duties during the time that Project Search is not in session. Number of hours to be determined by the program's needs, up to 19 hours per week.
- Perform other duties and responsibilities as assigned.
