Job Tile: Care Manager Intern
Reports to: Care Manager Supervisor

Position Job Requirements:
- Review referral information (assessments) for new youth with the assigned CM prior to the coordination of the initial meeting.
- Attend the Child and Family Team meeting with the assigned CM within the agencies time frames to gather additional information, assist with creating an ISP (individualized service plan) to address the youths/family’s needs, develop a rapport and identify additional team members both informal and formal.
- Assist the assigned CM with completing the individualized service plan within the agencies time frames.
- Assist the assigned CM with finding providers to work with the family based on services identified by the CFT.
- Support Individualized Service Plan goals and objectives.
- Document all contact, intervention and status updates as requested by the assigned CM and CMS and provide to the CM and CMS for review.
- Provide vacation coverage for Team in the absence of CM’s
- Assist Team with finding providers for youth as needed
- Monitor ISP by contacting providers to receive feedback regarding progress towards goals and support positive collaborations.
- Demonstrate professionalism and sensitivity when interacting with child/family as well as HPCMO staff.
- Meet with the CMS for weekly supervision
- Uphold appropriate levels of confidentiality
- Maintain respect for consumer rights.
