This role will support both the Brooklyn and Staten Island sites. This is a full-time, permanent, 40-hour per week role Mon-Fri, day shift w/ some evening and on-call as needed

Education Requirement: Minimum AA degree in Social Services or related field with at least 2-years relevant work experience required; BA preferred with experience working with at-risk youth preferably in a congregate setting, former case management and knowledge of child welfare is a plus.
* A Valid NYS Driver's License and access to personal vehicle is required for consideration
Job Overview: The Aftercare Life Coach is vital to the social service department and works in partnership with the case planning team, providing ongoing social/ emotional support to youth while developing individualized academic/ career development and life goals. This role is critical to the resident youth's successful long term life goals and independence. The Coach serves as an advocate/ supporter/ counselor/ mentor to and for the youth in all aspects of their life. Must be a team player with the ability to ensure needed services and supports are established for the youth's success and goal to independent living utilizing evidence-based models of service and care.
