Position Title: Education and Transition Specialist
Position Summary:
The Education and Transition Specialist provides essential support and independent living resources to Case Managers, with a focus on preparing youth (ages 17 and older) in foster care for successful transitions into adulthood.
Education:
-
High school diploma required
Experience:
-
Office experience preferred
-
Experience working with high-risk or foster care youth strongly preferred

Intermediate proficiency in Microsoft Word, Excel, and Outlook required
Licensure/Certification:
-
Must be at least 21 years old
-
Valid driver’s license and proof of auto insurance required
Key Responsibilities:
-
Promote a safe, community-centered, and evidence-based child welfare system across Kansas
-
Schedule and facilitate transition planning meetings for youth aging out of foster care
-
Gather and organize educational records and referral materials

Participate in case planning conferences to present and explain available DCF transition benefits

Prepare and distribute transition packets to youth and assist with their educational transitions

Support youth with applications for scholarships, Job Corps, and post-secondary education programs

Serve as a liaison between foster youth and school systems to ensure educational continuity

Assist with medical card applications and coordinate necessary documentation

Train internal staff on the Every Student Succeeds Act (ESSA) and related educational rights

Submit required reports, including monthly statistical summaries and updates
