JOB SUMMARY
The Population Health Wellness Coordinator (WC) is responsible for the success of all chronic disease related activities, classes and programs for the Rural Health Network. The WC will oversee the programmatic aspects of all assigned Network activities, including program coordination and development, program marketing, mentoring of Program Specialists, and facilitating collaborative projects in an effort to improve the health and wellness of the rural community. The WC is responsible for overseeing the day to day implementation of work plan objectives as outlined in awarded grants, as well as assisting the Executive Director with grant writing, reporting, and data collection. The position is grant funded for the duration of the grant periods.
SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS
- Coordinates all assigned grant programming including scheduling programs and events, data collection, and recruitment of participants, including day to day implementation of work plan objectives
- Ensure fidelity to work plan measures and evaluation plan by ensuring data is collected consistently and accurately.
- Provide oversight of community and programmatic events, including the delegation of planning and logistical details for events.
- Supervise the creation and posting of marketing material for Chronic Disease programing for the RHN, by supervising staff creation of material, planning content to be posted, and working with marketing department to ensure uniformity in messaging.
- Report to the Networks Executive Director and participate in leadership meetings to assist with the development of future network projects and goal setting.
- Coordinate with referral partners, including facilitating care navigation meetings when appropriate. Increases referrals to all assigned projects and initiatives.
- Assist Network Director with identifying and applying for future grant funding
- Work in and with the community, to host forums and events that gather community feedback and involvement.
- Attends both internal and external meetings as assigned

Hours: Full Time, 36 hours per week. Includes prescheduled weekends as needed
Salary: $27.54 per hour

Requirements:
EXPERIENCE
Required:
- Health care or health promotion credential or experience
- Experience working in health promotion in a rural setting, including an interest in advancing rural health
- Knowledge of and experience working in the area of chronic disease management and prevention
- Experience with event coordination/planning
- Ability to communicate effectively, both orally and in writing.
- Proficient in computer software programs including Microsoft Office
Preferred:
- 5+ years experience supervising / managing staff required
- 5+ years experience working on grant funded projects (grant reporting, data collection, etc.) preferred
- Experience with data collection, data analysis, and report writing
- Experience providing education/counseling related to health behavior change, and providing care coordination/management for a patient population preferred
- Resident of Wawarsing/Ellenville region or someone familiar with the region and available resources
- Customer service, communication, problem-solving, multi-tasking
EDUCATION
- Bachelor’s degree
- Master’s degree in Public Health or related field preferred
CERTIFICATIONS/SKILLS
- BLS certification required
- Ability to establish and maintain effective working relationships with care givers, other employees and the general public.
- Ability to read, interpret and follow detailed verbal, written and illustrated instructions.
- Effectively operate various office equipment including copier, fax machine, computer and phone system.
- Ability to be flexible, organized, and function well under stressful situations
- Must possess effective communication skills
