Key Responsibilities
- Support Provide peer mentoring and warm handoffs for patients or potential patient families who are expecting or have children up to the age of two (2) who are impacted by substance use and/or mental health challenges.
- Engage Facilitate patient follow-up care and connect them with essential services like housing, vocational rehab, and specialty care.
- Document Accurately record all patient interactions in the electronic health record system.
- Educate Lead one annual session of theFatherhood is Sacred/Motherhood is Sacredprogram and offer Narcan training to the community.
- Collaborate Work closely with a multidisciplinary care team to ensure holistic, patient-centered support.
- Advocate Use your recovery experience to help shape outreach strategies and promote community wellness.
Requirements
- Education High School Diploma or GED
- Valid Drivers License
- Experience At least 2 years in recovery from a mental health or substance use condition.
- Certification Peer Support Certification (or willingness to obtain within 30 days).
- Training Willingness to complete Narcan, Sacred Families, and Trauma-Informed Facilitation certifications. (we will provide these upon hire)
- Skills Strong communication, teamwork, and documentation abilities.
