Patient Outreach, Inbound and Engagement:

  • Custom Health (CH) associates will provide patient outreach and manage inbound calls as an extension of their pharmacy/health plan to engage a patient in the acceptance and provision of a defined service (i.e., medication synchronization enrollment, medication reconciliation, clinical interviews, MTM). CH associates will also coordinate with Custom Health pharmacists as directed.
  • Custom Health associates will provide services based on the needs of contracted clients. Clients may be MTM vendors, health plans, hospitals, physician clinics, home health agencies, pharmacies, PBMs, and technology/software companies.
  • Custom Health associates will proficiently interpret and appropriately document the contents of faxes and other communications received in a professional manner, as needed.
  • Custom Health associates will telecommute utilizing secure access technology.
  • Custom Health associates will work a full-time, 40-hour week between 9 am to 7 pm EST. This position may require flexibility, however, the hours of work completed must be during the company's hours of operation.

Requirements:

Qualification and Requirements:

  • Current state-issued Pharmacy Technician registration, certification, or licensure as required in state of residence.
  • National Certification is highly preferred (PTCB or NHA).
  • 2+ years of community pharmacy experience is preferred.?
  • Availability to work 40 hours a week within hours of operation:

Monday - Friday (times may vary depending on location)

  • Sales or call center experience is preferred.
  • Excellent communication skills with the ability to develop patient rapport telephonically.
  • Ability to work independently while being part of a collaborative (virtual) team.
  • Ability to preserve/protect confidential information.
  • Preferred experience with Medication Therapy Management programs and medication reconciliation/synchronization.
  • Data entry experience and the ability to type at a proficient speed. 50 or more words per minute is preferred.
