HEALTH COACH JOB DESCRIPTION

Position Title: Health Coach

Supervisor: Director of Patient Experience

FLSA Status: Non-Exempt

The Health Coach, under the direction of the clinical team, is responsible for overall health communication and activity, promoting and educating the population about behaviors that advance nutrition, physical activity, health and well–being. The Health Coach is also responsible for the interactive experiences that assist individuals to make appropriate health decisions. Will participate in education sessions, monthly newsletters, and additional patient engagement events. The Health Coach will use appropriate strategies and methodologies to facilitate behavior change and disease prevention. The qualified candidate must have a thorough understanding of diverse populations and how allied health fits into the healthcare spectrum.

ESSENTIAL FUNCTIONS:

1. Communicate and advocate effectively for health consumerism, health education and healthcare understanding

2. Meet with patients (in person, virtually, or over the phone) to discuss goals, progress and challenges

3. Translate scientific language and concepts into clear, simple information that is easily understood by patients and clients to maximize the potential for them to take action

4. Assess patient and client needs for nutrition, physical activity and health education, including change readiness

5. Help develop, implement, and improve patient education material and tools that encourage nutrition, physical activity and healthy behavior decisions

6. Conduct health education with patients and their families individually or in groups

7. Develop, monitor, and assess health communication interventions and policies

8. Assist with training in core competency and health area specific content and professional development

9. Work collaboratively with Vigilant Health Care Team members

10. Travel to service locations as assigned.

11. Plan and implement outreach and/or promotional events to recruit and/or engage patients

12. Support clinic operations and provide other services as needed.

MINIMUM POSITION QUALIFICATIONS:

• Graduate of approved professional school - Master's Degree preferred

• Certified Registered Dietitian, Certified Exercise Physiologist or Health Coach Certified

• Licensed to practice in the state of MS

• Minimum two years' work experience in a related field

• Ability to work with autonomy

9. Processing Information -- Compiling, coding, categorizing, calculating, tabulating, auditing, or verifying information or data.

10. Provide Consultation and Advice to Others -- Providing guidance and advice to management or other groups on technical, systems-, or process-related topics.

11. Training and Teaching Others -- Identifying the educational needs of others, developing formal educational or training programs or classes, and teaching or instructing others.

12. Establishing and Maintaining Interpersonal Relationships -- Developing constructive and cooperative working relationships with others and maintaining them over time.

13. Developing and Building Teams -- Encouraging and building mutual trust, respect, and cooperation among team members.

14. Evaluating Information to Determine Compliance with Standards -- Using relevant information and individual judgment to determine whether events or processes comply with laws, regulations, or standards.
